If you would like to request a Psychoeducational Assessment, Psychodiagnostic Assessment, or another type of evaluation, please send us a message using the form below.
Once we receive your message, our Intake Worker will contact you to discuss your needs and guide you through the next steps. Our Intake Workers are not therapists, but they can help determine which type of assessment is most suitable and provide information about availability and wait times.
If the assessment you require is not currently available, we can add you to our waiting list and suggest alternative resources when appropriate.

We currently have availability for Adult ADHD, Adult Psychoeducational and Adult Psychodiagnostic assessments.
We will reopen the waiting list for Child and Youth assessments in January, with appointments expected to be scheduled in Spring 2026.
